A lovely, attractive patient sought my opinion regarding tooth #11. This is a second crown she had made in 2 years. The first one was made because there subgingival caries according to her dentist. That crown broke and a new crown was just fabricated.
She is not happy with the aesthetics but that is not the most challenging part of this scenario.
Attached are the photographs of the patient's dentition as well as a close-up of #11. The x-ray reveals the restoration as well as the periodontal defect which probes 7 millimeters.
This very fastidious, attractive individual is concerned about how to proceed.
The endodontics appears to be well done and the tooth has a very long root. However, the aesthetics of a final restoration if it is to be redone, particularly following any periodontal treatment of #11 on the mesial, could result in a severely compromised outcome.
